Services Provided: Provide comprehensive solutions to ending homelessness and achieving self-sufficiency. The organization helps people reclaim their dignity and make lasting transformations in their lives. CCC’s approach addresses both the systemic drivers of homelessness and the individual factors that can reinforce it. We help our clients overcome barriers such as lack of affordable housing, health care and living-wage jobs, systemic racism, mental health challenges, chronic health conditions, substance use disorders, and time spent in the justice system.

Services: Support for reentry for individuals involved with Multnomah county probation and parole that would need support with finding housing and guide to get drug and alcohol treatment, social security navigation. Specialize in mobility and developmental disabilities.

JOIN: Connecting the Street to a Home
Services Provided: JOIN bridges the gap between homeless and housed members of the community. They provide outreach services to homeless individuals who remain outside the traditional social service system. The operational priority is to rapidly re-house individuals and families and stabilize them in their new homes. The services are not for individuals with homes who are at risk of losing housing. Day space Hours Mon/Wed/Thurs/Fri 12-4pm
Comments: Services include Outreach & Engagement; Housing Retention; access to Basic Services such as laundry, mail pick-up and access to DHS and the Outside In Medical Van and acupuncture; and Immersion Programs for people folks to learn about homelessness, poverty, and social injustice.
