Comments: Offers a comprehensive assessment of needs and recommendation of prescreened care settings. No eligibility requirements.
Services Provided: Residence where survivors receive training and support to prepare them for living in the community with the goal for residents to reach the highest level of independence possible.
Comments: Must be eligible for Medicaid. WC accessible. Uhlhorn Program is a service of ShelterCare. The program serves 39-50 brain injury survivors annually.
Windsor Place Inc.
Services Provided: Residential care facility providing rehab to adults with TBI. Offers a specialized program of skill development to enable survivors to become more independent.
Cost/Funding: Medicare, Medicaid, Worker’s Compensation and private pay.
Comments: Consists of three homes with a total of 15 residents. Average age of residents is approximately 35 years old. WC accessible. No motorized wheelchairs allowed. More admission criteria on website.
Services Provided: Provides low-cost accessible housing and support services to physically disabled adults. QUAD’s housing is designed to encourage self-reliance and self-sufficiency in daily life.
Cost/Funding: Residents pay 30% of their adjusted gross income for rent.
Comments: Must be low-income, primarily rely on a wheelchair for mobility & have a desire & ability for independent living. Must be able to direct their own care.
Services Provided: Avamere has more than forty senior living facilities in Oregon and Washington. They offer a full range of retirement and health services in residential living setting. Also offers rehabilitative, long-term and outpatient care.

Kampfe Management Services
Services Provided: A community based residential rehabilitation program for adults with traumatic brain injury. The program is tailored for each individual and focuses on developing independent living skills and behaviors that support it.
Cost/Funding: Private pay and Medicaid.
Comments: Residents need to be independent with basic self-care and mobility, including ability to use stairs. Services also include vocational training.
Homeward Bound Rehabilitation Services
Services Provided: Residential rehabilitation services for survivors of traumatic brain injury in a rural community. Goal of the program is for each resident to obtain the highest level of personal recovery possible, through physical, emotional, and spiritual support.
Cost/Funding: Private pay.
Comments: Long-term and short-term residential plans. Adults residents only. Wheelchair accessible.
