Services Provided: At Brain Rehab Network, we think differently about helping adults who have a brain injury. Keeping the person at the center of care, we work tirelessly toward functional independence and quality of life. We offer Comprehensive Outpatient Rehabilitation Program (MD, Psych, OT, PT, SLP), Transitional / Supported Living Program, Home / Community-based Program.

Bridge to Independence

Services Provided: Therapeutic, occupational therapy based environment where a TBI/stroke person can find community and healing. Offers goal-directed individual and group activities that engage each participant in physical, cognitive, and social skill building, as well as recreational activities.  Must be 18 and older.

Comments: Must be 18 or older. Directed by a Licensed Occupational Therapist who also provides individual assessments and service plans. Wheelchair accessible.

Services Provided: Private practitioner, speech-language pathology, cognitive-communication therapy/rehabilitation

Cost/Funding: Currently private pay with sliding scale. Superbill provided for out-of-network benefits. FSA/HSA and credit cards accepted.

Comments: Kate is a licensed speech therapist and certified brain injury specialist offering a collaborative, whole-person, mentorship approach to speech therapy. She has personal experience with cancer and “chemo brain” and often works with people experiencing cognitive changes resulting from cancer treatment, concussion, and other brain injuries. In addition to individual therapy and coaching services, Kate offers occasional group experiences, including expressive writing and aphasia groups. She is currently providing most services remotely via telehealth, with occasional in-person sessions as needed.

Independent Living Resources

Services Provided: An aging and disability resource connection partner for finding resources, limited housing resource referral, employment, financial assistance and health care. The four core services are: 1) Advocacy, 2)Information and Referral, 3) Peer Counseling and 4) Skills Training. ILR provides opportunities for those with disabilities to talk one-on-one or in a group about life experiences, offering individual peer support in person, via phone and virtually. Available for all ages.

Cost/Funding: Services are free. Limited to Multnomah, Clackamas or Washington County. If calling from another county they can do an informational referral conversation. Braille Voter Guide available, please call.

Providence Rehabilitation Services

Services Provided: Our rehab programs are designed to go beyond the work of physical medicine and bridge your transition from therapy to recovery. We promote independence in the rehabilitation process and place a strong emphasis on measurable outcomes and supporting your work to improve physical function. Outpatient and inpatient rehabilitation services with physical and occupational therapists who specialize in treating balance and vision.

Cost/Funding: Most health insurance carriers provide coverage of rehabilitation needs resulting from vision and balance impairment. Insurance benefits review and private pay options are available. Providence also offers financial assistance and alternative payment options.

Providence Portland Hospital Acute Rehabilitation Center

Services Provided: Comprehensive care and acute inpatient rehabilitation center that offers intensive level of physical, occupational and speech therapy therapy following a serious injury. in order to regain self-confidence, improve functional skills, and achieve maximum physical, cognitive, language and psychosocial skills.

Comments: Requires MD referral and an eligibility evaluation by a Providence Portland Hospital Acute Rehabilitation Center staff. Must be medically stable and able to participate in and benefit from three hours of therapy daily.

Services Provided: Offers neurorehabilitation services that are individualized to a person’s needs. These services are provided in one’s real-life settings, focusing on meaningful tasks that lead to functional independence. Provided in the home, community and workplace settings. .
