Brain Rehab Network

Services Provided: At Brain Rehab Network, we think differently about helping adults who have a brain injury. Keeping the person at the center of care, we work tirelessly toward functional independence and quality of life. We offer Comprehensive Outpatient Rehabilitation Program (MD, Psych, OT, PT, SLP), Transitional / Supported Living Program, Home / Community-based Program and Residential programs.

Homeward Bound Rehabilitation Services

Services Provided: Residential rehabilitation services for survivors of traumatic brain injury in a rural community. Goal of the program is for each resident to obtain the highest level of personal recovery possible, through physical, emotional, and spiritual support.

Housing Connections

Services Provided: A service of 211info designed to help people locate affordable housing in the Portland/Vancouver metro area. This program is funded by the Portland Housing Bureau and the Clark County Department of Community Services.

Independent Living Resources

Services Provided: An aging and disability resource connection partner for finding resources, limited housing resource referral, employment, financial assistance and health care. The four core services are: 1) Advocacy, 2)Information and Referral, 3) Peer Counseling and 4) Skills Training. ILR provides opportunities for those with disabilities to talk one-on-one or in a group about life experiences, offering individual peer support in person, via phone and virtually. Available for all ages.

Cost/Funding: Services are free. Limited to Multnomah, Clackamas or Washington County. If calling from another county they can do an informational referral conversation. Braille Voter Guide available, please call.

Windsor Place Inc.

Services Provided: Residential care facility providing rehab to adults with TBI. Offers a specialized program of skill development to enable survivors to become more independent. The emphasis for our TBI residents is focused on brain-injured adults re-learning basic daily activity skills with the goal of maximizing their independence capabilities to the fullest degree. Consulting occupational and physical therapists, as well as nursing and behavioral specialists assist our trained staff to be mindfully in tune with each of our resident’s needs.

Cost/Funding: Medicare, Medicaid, Worker’s Compensation and private pay.

Comments: Consists of three homes with a total of 15 residents. Average age of residents is approximately 35 years old. WC accessible. No motorized wheelchairs allowed. More admission criteria on website.
